Determining Your Eligibility for Support Schemes 

We provide a range of home and community-based services to meet the diverse needs of seniors to support ageing in community. 


Our Severe Disability Assessment service allows seniors to know if they have grounds to make a claim from severe disability insurance schemes.

Some of these schemes include: 

  • CareShield Life 
  • ElderShield 
  • MediSave
  • Care Interim Disability Assistance Programme for the Elderly (IDAPE) 

The assessment can be performed during a house call or at our TOUCHpoint@AMK. 

As this is a paid service, our fees for the service is as follows: 

  • House call
    • $250 per assessment (single visit)
    • $250 per assessment (single visit)
    • $100 per assessment (multiple visits) 
  • Centre-based
    • $100 per assessment

Please note that these fees will be fully refunded if the senior is determined to meet the criteria for severe disability. 

Who It’s For

This service primarily caters to severely disabled seniors.  Individuals are considered severely disabled if they are assessed to be unable to perform at least three out of six activities of daily living (ADLs): 

  • Dressing 
  • Showering 
  • Feeding
  • Toileting 
  • Transferring  
  • Mobility 

Seniors looking to use our service should also meet these requirements: 

  • Wish to make a claim from their severe disability insurance plan 
  • Have received a letter from the Agency of Integrated Care (AIC) informing them to go for a periodic re-assessment to continue receiving payouts 
  • Have the following supporting documents  
    • NRIC 
    • Medical reports and/or doctor’s memo
    • Completed claim form for supplements (if you are making claims from private insurers) 
    • Periodic review letter from AIC (if applicable) 

Where do the assessments happen? 

All centre-based assessments take place at the Day Rehabilitation Centre, located at TOUCHpoint@AMK 433.

We also offer house-calls for seniors who live around our Home Care centre boundaries, namely:  

  • Ang Mo Kio  
  • Jurong 
  • Toa Payoh 

What You Can Expect

How to Apply

The Severe Disability Assessment is performed by Ministry of Health-accredited assessors to evaluate your ability to perform the six activities of daily living (ADLs). 

Before commencing the assessment, our assessor will ask if you have any past medical history or existing medical conditions. 

You will then be asked to demonstrate how ADLs are performed. 

If required, your caregiver will also be asked to show how he/she assists you as you perform the activities.
