We provide a range of home and community-based services to meet the diverse needs of seniors to support ageing in community.
Our Severe Disability Assessment service allows seniors to know if they have grounds to make a claim from severe disability insurance schemes.

As this is a paid service, our fees for the service is as follows:
Please note that these fees will be fully refunded if the senior is determined to meet the criteria for severe disability.
This service primarily caters to severely disabled seniors. Individuals are considered severely disabled if they are assessed to be unable to perform at least three out of six activities of daily living (ADLs):

Where do the assessments happen?
All centre-based assessments take place at the Day Rehabilitation Centre, located at TOUCHpoint@AMK 433.
We also offer house-calls for seniors who live around our Home Care centre boundaries, namely:
The Severe Disability Assessment is performed by Ministry of Health-accredited assessors to evaluate your ability to perform the six activities of daily living (ADLs).
Before commencing the assessment, our assessor will ask if you have any past medical history or existing medical conditions.
You will then be asked to demonstrate how ADLs are performed.
If required, your caregiver will also be asked to show how he/she assists you as you perform the activities.
