Bringing Essential Care to Seniors’ Homes
We provide a range of home and community-based services to help seniors gracefully age within their community.
For seniors with mobility issues, it can be challenging to make their way to and from medical appointments safely.
Our home medical and nursing service is ideal for seniors with stable chronic conditions but are less mobile and have difficulty going for medical appointments. They can rest and recover at home while still receiving essential medical care.
Medical Support We Provide
Seniors who use our service typically require long-term medical and nursing care due to chronic diseases and/or disabilities. Our team can assist them with:
Home medical
- Chronic disease management
- Referral for specialists’ care and consultation
- Prescription of medication
- Client and caregiver education
- Minor procedures such as wound debridement, intra–articular injections
- Ordering for investigations
Home nursing
- Administration of injections, medications, tracheostomy, and drainage tubes, etc.
- Wound management and stoma care
- Management and changing of urinary catheters, and nasogastric tubes
- Monitoring of vital signs and medication condition
- Client and caregiver education
- Monitoring of medication compliance
Who It’s For
Our home medical services are available for seniors who:
- Are homebound
- Have chronic conditions
- Have disabilities that require long-term medical care
Our home nursing services cater to seniors who:
- Are considered frail
- Are homebound
- Have been diagnosed with chronic conditions or disabilities, and require care that must be provided by a trained nurse
Our Home Medical and Nursing service is only available to seniors residing near TOUCH Home Care centres in Ang Mo Kio, Bishan, Sin Ming, Yishun, Toa Payoh, Whampoa, Bendemeer, Potong Pasir, Jurong, Boon Lay.

How to Apply
You may do one of the following:
- Approach a doctor at a hospital or polyclinic to submit an online application for you
- Contact the Agency for Integrated Care (AIC) at 1800 650 6060 or email enquiries@aic.sg
- Visit the AIC Link branch nearest to you
What You Can Expect
Costs
Our service support team will contact you to inform you of the charges. Subsidies may be available. Means testing will determine your eligibility and the level of subsidy. The means test is based on household income per capita or annual value of your residence.
Initial assessment
Our doctor or nurse will visit you to assess your home setting and discuss the type of medical or nursing services you may require.
Your care plan
After the assessment visit, you will be advised on the appropriate frequency of follow-up sessions required.
