Home Therapy

Supporting Independence with Therapy Intervention  

We provide a range of home and community-based services to meet the diverse needs of seniors to support ageing in community. Home therapy interventions allow seniors to regain their mobility, so that they can continue to live safely and independently at home and in the community. 

Our qualified occupational therapists and physiotherapists will work with individuals and their caregivers to recommend a customised intervention plan based on their personal goals. 

Occupational Therapy at TOUCH 

We help seniors restore their ability to function independently by: 

  • Training them in activities of daily living skills, so they can once again manage their own basic physical needs, including eating and grooming 
  • Training them in cognitive strategies to cope with the demands of performing daily activities safely 
  • Recommending home modifications to improve safety and accessibility 
  • Prescribing assistive devices and/or powered mobility aids if necessary 

Our occupational therapists will also train caregivers so that they can better support their charges. 

Physiotherapy at TOUCH  

For seniors affected by injury, medical conditions, or disability, we aim to restore their mobility and physical function by:

  • Providing treatment that may include gait retraining and manual therapy  
  • Prescribing exercise plans, pain management strategies, medication, and ambulatory aids 

As with occupational therapy, our physiotherapists will also train caregivers so they can provide better support for their charges. 

Who It’s For

Our Home Therapy service is only available to seniors residing near TOUCH Home Care centres in Ang Mo Kio, Bishan, Sin Ming, Yishun, Toa Payoh, Whampoa, Bendemeer, Potong Pasir, Jurong, Boon Lay.

Individuals must meet these requirements: 

  • Assessed to require home therapy
    • This assessment must be done by a therapist registered with the Allied Health Professions Council, an advanced practice nurse, or a medical doctor
  • Not eligible to receive therapy at an outpatient rehabilitation centre or community hospital  
  • Has potential to improve through home rehabilitation

  • Visit the AIC Link branch nearest to you 

What You Can Expect

Costs

Our service support team will contact you to inform you of the charges. Subsidies may be available. Means testing will determine your eligibility and the level of subsidy. The means test is based on household income per capita or annual value of your residence.  

During the sessions

Therapy sessions will be conducted in your home on an appointment basis. 

  • For the first visit, the therapist will assess your needs and develop therapy goals with you. 
  • The frequency of sessions will depend on your progress. 
  • The therapist will also work with you to develop a discharge plan when you have been assessed fit for discharge.
