Optimising the Potential of Our Seniors 

Funded by the Tote Board Community Health Fund, our Reablement Programme proactively aims to restore seniors’ day-to-day functional abilities. Through this 12-week home-based initiative, seniors will regain the capacity to independently manage all or some aspects of their daily tasks in a safe manner. 

We focus on a “doing with” rather than a “doing for” approach with our seniors. This empowers them to become more independent, confident, and enjoy a better quality of life.

Who It’s For

  • Seniors aged 50 years or older
  • Stable medical conditions 
  • Able to remember and apply new information 
  • Willing to participate in 12 consecutive weeks of training 

What You Can Expect

Reablement involves a 12-week home-based intervention programme. The team running the sessions is made up of a:

  • Care coordinator
  • Care associate 
  • Occupational therapist 
  • Physiotherapist 
  • Registered nurse 
Frequency

The sessions will be conducted in your home on an appointment basis, for one hour, twice a week.  

First visit

During the first visit, the reablement team will: 

  • Conduct a holistic assessment to understand your current health, functional and social goals
  • Develop a care plan with you based on your identified priorities 
Costs

Our team will inform you of the charges before you start the programme. 

Subsidies may be available. Means testing will determine your eligibility and the level of subsidy. The means test is based on household income per capita or annual value of your residence. 

During the programme

Once the sessions start, our team will closely monitor your training and conduct assessments to evaluate your progress.
