TOUCH Cluster Support (TCLS) aims to enable vulnerable elderly, who have little or no family support, to age-in-place in the neighbourhoods of Kallang/Novena and Yishun/Sembawang regions.
The TOUCH Cluster Support team, which comprises community social workers, will make home visits to assess the well-being of frail and vulnerable seniors, particularly those with limited family support referred by community partners or through outreach in the neighbourhoods. With this layer of community support, it will ensure that this group of seniors will not fall through the cracks and continue to age with grace and dignity.
The social workers also provide case assessment and counselling besides coordinating healthcare services such as day care services, home nursing, personal care services, meals-on-wheels and transport service for medical appointments, as well as assist in transitional care from hospitals back to their own home.
The other key component is the Caring Assistance from Neighbours (‘CAN”) programme where the CS team will help to recruit neighbours to take on the role of a caregiver to provide friendship and serve as a look-out for these seniors by checking on their well-being.
Through its work, TCLS hopes to see:
- Frail and vulnerable seniors being able to access appropriate services and age-in-place
- Neighbours being empowered to care for the frail and vulnerable seniors
- Together with the Senior Activity Centres, the CS team creates a strong social and care support network for the seniors

Piloted the Grief Support Group to help seniors process the psychological impact caused by non-death causes, such as the loss of one’s functioning ability.
