How well do your loved ones really know you? What happens when you are terminally ill or become mentally incapacitated? Will your healthcare professionals or loved ones know how to decide on important medical or care decisions affecting you?
Advance Care Planning (ACP) helps you communicate and/or document your values and wishes regarding your future medical care.
ACP is not a legal document. But it is never too late to document your wishes, personal beliefs and values early with the help of an ACP facilitator and make known your healthcare preferences in different medical scenarios.
If you are above 21 years old, you can make an ACP regardless of your health condition and change it at any time.
1. Book an appointment by calling 9653 1076 or 6317 9996 during office hours (Mon to Fri, 9am to 6pm), or email [email protected] if you need more information.
2. ACP Session (60 mins): Tele-ACP OR one-to-one sessions at TOUCH centres located at Bukit Merah and Ang Mo Kio
3. Completion and uploading of ACP documentation with the National Healthcare Record office by ACP Facilitator
