Job Summary:
You will be responsible for organizing and conducting outreach to increase mental health and demential awareness and promote early identification. You will support clients who are at risk of mental health issues and/or demential by linking them to the necessary support and resources. You will continue to monitor these clients in the community.
Principal Responsibilities and Duties:
- Provide care coordination and service linkage to clients with or who are at risk of mental health and/or dementia, with the appropriate resources and support including caregiving support.
- Plan and conduct outreach to the community and stakeholders including faith-based, corporate and community organisations to promote mental wellness and mental health awareness.
- Plan and conduct targeted mental health and dementia case finding/screening to promote early identification and facilitate timely support.
- Plan and organize structured preventive activities and/or programmes for the community, those at risk of mental health conditions and clients.
- Responsible for maintaining client records that are accurate and timely for submission, in accordance to the programme requirement.
- Support in the development of service planning document and manual including set up requirements (infrastructure, manpower, training, recruitment) and processes (outreach, screening, mental health information, servie linkage & follow-up)
- Ensure attendance in training and supervision sessions, where relevant, as part of the staff comptetency required by them programme.
Requirements
- Degree/Graduate Diploma in Social Work, Psychology, Counselling; registered with SASW is preferred
- Experience with mental health work
- At least 2 years experience in case management and group work
